Thank you for agreeing to participate in this survey.  This survey is being provided to members of the Match-e-be-nash-she-wish Band of Pottawatomi Hazard Mitigation Plan (HMP) Committee for Public Health Emergencies, sponsored by the Gun Lake Tribe (GLT) Health and Human Services Office.  
The survey is located at the project web site:  Please, select the "First Survey" option on the left to access the survey and answer online.  If possible, please answer all questions and complete the survey by the end of the day, Tuesday September 12.

The objective of this survey is to review a ranking of hazards in preparation for an interactive workshop that will prioritize the hazard planning and mitigation process.  For your information there is a description of each hazard at the project web site: select "Hazard Definitions".

In your answers, please feel free to describe any hazard that you may consider significant (you are not limited to the list of hazards).  When determining significance, please consider severity of impact, probability of occurrence, and preparedness of the GLT to respond.  We are not requesting that you look at historical trends, or research existing response capabilities, but are looking to understand which hazards you think that the GLT would consider significant enough to require consideration in the planning and mitigation process.

The second part of the survey asks you to rank the top hazards from the current Plan, and then each of the remaining hazards so that they can be discussed at the workshop.   The final section asks you to rank critical assets.  Please indicate how important each of these are for the GLT to address by placing a check in the appropriate box (select only one ranking per hazard).  When determining importance please consider the negative consequences of the hazard on the population, economy and environment of the GLT service area.
